Because I assume the vacuum cavity of the vagina will cause relatively little tear to the condom, yet rubbing it outside involves greater range of movement. Plus the hand might wipe away some lube?

No-no-no - Vagina is not vacuum at all.
I would say it may be safer in vagina as the secretion during excitement acts as a lubricant, but condom is designed to withstand the thrusting action of male’s penis. Not all condoms have lubes too. So don’t worry.

The vagina is not a vacuum, so don’t worry about that.
You do want to make sure you are lubricated enough. Friction without enough lubrication could cause breakage of the condom.
